* Pursuant to the Regulation (EU) 2016/679 of the European Parliament and of the Council of 27 April 2016 on the protection of natural persons with regard to the processing of personal data and on the free movement of such data, and repealing Directive 95/46/EC (General Data Protection Regulation, GDPR) (Official Journal of the European Union, L 119, 4 May 2016) and the Act of 10 May 2018 on the Protection of Personal Data (Journal of Laws of 2019, item 1781), I hereby consent to the processing of my personal data by the Foundation for Polish Science with its registered office at Ignacego Krasickiego 20/22, 02-611 Warsaw (acting as the Data Controller), in particular, to organise the IRAPConference and data archiving. The data will be processed before and during the Conference and during the period of storage of financial, accounting and reporting documentation regarding the Conference.

At the same time, I acknowledge that I am entitled to the rights set out in Article 13(2b) of GDPR with respect to the content of my personal data processed by the Foundation for Polish Science, in particular, the right to request from the Data Controller access to and rectification or erasure of personal data, restriction of processing concerning the Data Subject, or to object to the processing of data, as well as the right to data portability. I also have the right to lodge a complaint with a supervisory authority (https://uodo.gov.pl/) and to withdraw my consent at any time, without affecting the lawfulness of processing based on consent before its withdrawal.

Submitting my personal data is voluntary, but necessary for the implementation of the above purpose. My data is not subject to automated decision-making. I am aware that my personal data may be disclosed to public or private entities if such obligation results from the generally applicable provisions of law.
